In the today’s era of ageing population, changing lifestyle, and medical innovations have drastically changed the number and group of people that have to undergo cardiac surgeries. The current group of patients is much older and is in a later stage of their disease which often translates into complex surgeries requiring hours of endless procedures. Not only this, but the ageing patients usually suffer from other diseases like diabetes, renal failure, previous strokes, which makes the matter worse. Due to these complex surgical requirements surgeons are faced with new challenges each time in the operation theatre. The primary objective in any cardiac surgery is to ensure adequate patients grafts while simultaneously ensuring least myocardial damage. Research conducted by scientists aimed at identifying various adverse affects of cardiopulmonary bypass (CPB) has gained much attention in the recent time. Considerable resources are being redirected to ensure the safety of CPB and develop new strategies that ensure myocardial preservation. One trend that surfaced was direct myocardial revascularization without CPB that is referred to as beating heart surgery. There were many risks involved with the use of CPB. Beating heart surgeries tends to skip these risks and allows patients to undergo surgeries that otherwise might not have been able to.

There are various beating heart surgeries. These primarily include off pump coronary artery bypass, minimally invasive direct coronary artery bypass and minimally invasive surgery. These surgeries require numerous equipments to ensure a successful procedure. CPB and perfusion machines are kept on stand-by mode in case they are required any time during the operation. Other equipment includes endoscopy cart, CO2 tank, and pressure bag of sodium chloride that is used for the blow/mister. Multifunction electrode pads are essential in case perfusion machines are needed anytime during the entire procedure. The monitoring procedure includes electrocardiographic monitoring (ECG), transesophageal echocardiography that is used to detect wall motion abnormality and optimize preload.

The key drivers for beating heart surgery include increasing demand for minimally invasive procedures, improving per capita income, rising number of heart attacks and strokes due to ageing population, unhealthy stressful lifestyle of consumers, obesity oriented issues and technological advancements. The end user segmentation includes public, private hospital and clinics and other facilities (defense hospitals, medical colleges). Depending on geographic locations, beating heart surgery equipments market is segmented into seven key regions. These are North America, Latin America, Eastern Europe, Western Europe, and Asia Pacific excluding Japan (APEJ), Japan, and Middle East & Africa. The global beating heart surgery equipments market is expected to show strong CAGR growth in the coming decade with expectations that the growth in Asia Pacific region would be the highest.

Mitral Valves Shaping Future Of Cardiac Valve Repairs Market Outlook: Ken Research

According to the study, ‘CARDIAC VALVE REPAIRS-MEDICAL DEVICES PIPELINE ASSESSMENT, 2018, for many, valve tightness can be relieved during a procedure called balloon valvuloplasty which is done as part of a cardiac catheterization which is less invasive than general surgery or open heart surgery. Commissurotomy is used for narrowed valves, where the leaflets are thickened and sometimes stuck together. The surgeon opens the valve by cutting the points where the leaflets meet. New progress in medical science has been improving the scenario considerably, evident by developments in treatment of ailments such as mitral regurgitation.
Today, there is an immediate concern regarding heart diseases which are on the rise owing to lifestyle imbalances. There are several heart valve ailments, all of which reduce the efficiency of the heart by reducing functionality of the valves. Options are available today that can resolve valve ailments either by repair or by replacement. When possible it is generally best to repair a valve and preserve a person’s own tissue in the heart. However, if the tissue is too damaged, a replacement valve may be used from another human heart, an animal or a manufactured mechanical valve.
Mitral regurgitation is the most common valvular disease and significant severity levels of mitral regurgitation are found in a few people above the age of 65 years. There are several features of the mitral valve for instance, its saddle-shaped non calcified annulus and irregular leaflet geometry, that make a transcatheter approach to repair or replace the valve more challenging compared to the aortic valve. New transcatheter minimally invasive technologies are being developed to address mitral valve disease in patients deemed too high a risk for conventional open-heart surgery. Several devices are under investigation for mitral valve repair, targeting the mitral valve leaflets, chordae tendinae and mitral annulus. The MitraClip device is the only Food and Drug Administration (FDA) approved device to treat mitral regurgitation by targeting the mitral leaflets. Currently there are more than ten different transcatheter mitral valves that are in various stages of development and clinical trials. These transcatheter mitral valves can be delivered either through a transseptal, transapical, transaortic, or left atriotomy approach.
Many big medical companies are exploring the prospects of cardiac valve treatment. US based Abbott Laboratories is a healthcare company that has recently been part of major developments in mitral valve treatment. In May 2018, Abbott announced favorable outcomes from the first hundred patients treated in a global study of its Tendyne Transcatheter Mitral Valve Replacement System (TMVR) system, the first and only mitral replacement valve that is repositionable and fully retrievable to allow for more precise implantation to improve patient outcome. This trial is the largest study of a transcatheter mitral valve replacement device to date.
Cardiac valve treatment is proving to be a flourishing research sector with more and more developments taking place. Repairing cardiac valves is increasingly incorporating new research evident by the developments in treatment of mitral regurgitation. New devices are being developed and certified by reliable establishments such as the US based FDA and many more are in the making. Global giants like Abbott are also showing their interests in heart valve treatment evident by their recent research project on transcatheter mitral valve replacement.

Changing Landscape of United States Prosthetic Heart Valve Industry: Ken Research

Heart valve disease occurs when one of the four valves in the heart function properly. This leads to reduced flow of blood through the heart. Congenital heart deficit at birth is the resultant of malfunctioning heart valve in pediatric patients. In the United States, significant numbers of babies are born with congenital heart valve defects. Most of the babies which suffer from this condition have to undergo heart valve surgery and replacement heart valve surgery. The prime advantage of mechanical valve over tissue valve is that they do not get degraded and rarely need to be replaced over concerns regarding operating. Mechanical valves are made from either pryolytic carbon or titanium that is coated with pryolytic carbon. Pryolytic carbon is an ingredient in many medical types of equipment. Due to their durability, mechanical valve are primarily used for correcting heart valve related condition in younger patients as compared to elder population. The newer models focus on an open-and-close valve, which allowed for central blood flow.
According to the study, ‘UNITED STATES PROSTHETIC HEART VALVE PROCEDURES OUTLOOK TO 2025’ some of the procedures and products that are currently available in the United States include Cardiovascular Procedures, Cardiac Assist Procedures, Cardiac Rhythm Management (CRM) Procedures, Cardiovascular Surgery Procedures, Clot Management Procedures and Interventional Cardiology Procedures. Some of the companies that manufacture heart valve replacement and repair devices are ATS Medical Inc., Boston Scientific Corporation, CarboMedics Inc., Medtronic Inc., and Sadra Medical Inc.
In March 2018, The United States Food and Drug Administration approved a small heart valve that could be used for the treatment of newborn patients. The agency’s approval market the smallest mechanical heart valve to be approved in the entire world. The approval was given for the Masters Series Mechanical Heart Valve with Hemodynamic plus (HP) Sewing Cuff to include the 15-mm valve size. The master series Mechanical Heart Valve is designed for usage in aortic/mitral position. It is a rotatable two leaflet valve. Just like a patient’s valve, the two leaflet design consists of two semi-circular discs that open and close in relation to blood pressure changes during the heartbeat. The current approval provides smaller patients another treatment option by expanding the size of valves available. The first Master Series Mechanical Heart Valve was first approved in 1995 especially for patients with malfunctioning aortic or mitral heart valve. The US FDA had previously approved this device for replacements in implanted aortic or mitral prosthetic heart valves. The FDA evaluated clinical data from 20 pediatric patients ranging from the age of 1.5 week to 27 months. The study revealed that surgical heart valve treatment in these age groups resulted in significantly lower probability of facing a heart valve related issue and simultaneously improved the probability of survival.
The United States prosthetic heart valve Industry is expected to show a positive growth in the coming 5 years. Many companies are coming up with technological improvements and innovations to improve the efficiency and effectiveness of these devices. Government regulations in the States have also supported the growth of this segment which translated into a strong base for the companies.
